Ordering Your Copies of PHR
20,000 copies of the document were printed in September 2005 by King Edward Memorial Hospital for Women (KEMH). These documents are provided free of charge to all General Practitioners referring pregnant women to KEMH. Please see the table below for details of the documents and costs where applicable.
To order the following resources please complete the order form and send to the Obstetrics and Gynaecology Clinical Care Unit at King Edward Memorial Hospital for Women.
Following receipt of the order form an invoice is raised when appropriate, the PHRs are forwarded to the purchaser by mail with accompanying plastic envelopes and documentation. Payment is requested within 30 days of receipt of goods.
